PAYMENT INFORMATION

If you are a self-pay patient we require that you pay at the time of your visit.

Patients with insurance are required to pay their co-pay before being seen.

We accept Visa, Master Card, American Express and Discover

 

INSURANCE

Prior to your appointment please check with your insurance company and make sure you are covered for the requested exam and that we are providers for your insurance company.

As a courtesy our billing office will bill your insurance.  However, if we are not a provider for your insurance company we ask that you pay for the visit at the time of your appointment.

Please bring a current insurance card to every appointment.
