Dear New Patient,
We would like to thank you for selecting PHASE II for your gynecology needs. Our goal is to provide exceptional medical care and you can help us by filling out the enclosed forms. We have designed a thorough "Initial Health History Questionnaire" which must be completed and returned before your first visit. We also request that you fill out and return the registration forms. Please allow plenty of time for your forms to be processed through the mail. If your appointment is soon, please bring the completed forms with you. If you do not bring them, you will need to arrive 30 minutes early to complete the necessary paperwork. Please give 24 hour notice if you must re-schedule or cancel your appointment to avoid a no-show fee.
Upon arrival at our office, you will be given information about arbitration and an Arbitration Agreement to sign. Arbitration is a method of resolving issues or claims without going through the court system. When a problem arises, we want you to come to us and address it. Most concerns have historically been resolved by simply working together to find a reasonable outcome. We are satisfied that arbitration doesn't in any way jeopardize your right to be compensated if you are harmed by the treatment process.
Our new office location is 348 East 4500 South, Suite 200. The building is set back off the street in the Towers at 45th complex. Parking is available on either the west side in marked "348" spaces or on the east side. Access to our office is only available on the main northwest corner door. Handicap parking is available on the west side only.
Dr. Graul's and Dr. Hurst's practices are focused on pelvic surgery and treatment of complex gynecologic problems including abnormal bleeding, urinary incontinence, and pelvic organ prolapse. Our nurse practitioners, Shirley Bleak, APRN, Elizabeth Collins, APRN, Holly Stewart, WHNP, Ronda Lucey, APRN and Rebecca Gatesman, APRN, have decades of women's health care experience and provide all our routine gynecologic exams and pap testing as well as birth control counseling and hormone replacement therapy. They see patients for urgent problems like bladder infections and vaginitis, and can evaluate and treat any gynecology-related problem. They will refer you to one of the doctors if a serious problem is encountered.
